Any young person wishing to participate in the activities undertakes to act in a respectful manner towards the other participants and all the supervisory staff involved as well as towards the equipment and materials used. *Any conduct deemed inappropriate will result in the expulsion of the young person.*

I authorize the organizers to act on my behalf in the event of an emergency. If necessary, the cost of transport by ambulance will be assumed by the parents. The organizers of the event are not responsible for any accidents occurring during practice or the final. I authorize the recording of photo or video images of my child and the use of this material by the organization for promotional purposes.
